Bring the completed forms, along with the necessary information as described below, to the Rhode Island Lottery, 1425 Pontiac Avenue, Cranston, RI 02920.

2 copies of applicant’s driver’s license.
$50.00 Non-Refundable Application Fee Payable to The Rhode Island Lottery.
Rhode Island BCI Check*.
Copy of Permit to Make Sales at Retail License.
Voided check (If opening checking rather than savings account).
Front page and waiver form must be completed and notarized.
List two business references (vendors) where you buy your supplies, products, etc.
W-9 form also needs to be completed.
